Main Duties:
- Developing and implementing personal care plans to address the specific needs of geriatric patients.
- Collaborating with multidisciplinary teams, including nurses, therapists, and social workers, to provide holistic care for elderly patients.
- Managing chronic medical conditions commonly seen in the elderly, such as dementia, arthritis, and cardiovascular diseases.
- Providing counselling and support to elderly patients and their families regarding medical decisions and end-of-life care.
- Conducting ward rounds and outpatient clinics to assess and manage geriatric patients in both hospital and community settings.
- Participating in educational activities, including teaching, and mentoring medical students and junior doctors.
- Adhering to NHS protocols, guidelines, and ethical standards in all aspects of patient care.

The frailty team are based in the emergency department and are dedicated to identifying and supporting frail patients as soon as they enter the hospital for acute illnesses and incidents. The hospital itself offers a wide range of services in their emergency department, maternity unit, pediatric ward, and surgical suites to improve the care of any patient that enters the front doors. The facility also continually invests in modern medical equipment and technology to ensure efficient and accurate diagnoses and treatments.

Requirements:

- Full GMC with a license to practice.
